There actually was a census schedule with this name. There was also one taken in 1880, though it was a little more
politically correctly titled "Schedules of Handicapped, Dependent & Delinquent Inhabitants of Illinois".
This just goes to show what you can find in the Illinois State Archives with a little digging!
The original census-taker did not fill in the "Which Disease" column, which is more than a little annoying - not to mention the fact that he couldn't be bothered to write down my relative's first name. <Sigh>
