Declaration for Dependent Pension of a Mother or Father

Under Act of Congress Approved June --------, 1890

(This must be Executed before a Court of Record or Some Officer Thereof having Custody of the Seal)

State of Illinois, County of Johnson, SS:
On this 8th day of July, A.D. one thousand eight hundred and ninety -----, personally appeared before me, James W. Gore, Clerk of the County Court, a Court of Record in and for the County and State aforesaid, Hezekiah L. Lambert, aged 76 years, who, being duly sworn according to law, makes the following declaration in order to obtain the Pension provided by Acts of Congress approved June 27, 1890. That he is the father of William Lambert, who enlisted under the name of William Lambert, at Vienna, Ill on the 31st day of August, A.D. 1861 in Co. F, 31st Ill. Vols. Inft. in the war of the rebellion, who was killed at Vicksburg, Miss. on the 4th day of July, 1863; that said son Wm. Lambert left neither widow nor child under sixteen years of age surviving; that the said declarant was married to the mother of said son at _____________, on the _________ day of __________, A.D. 1833, by __________________; that he is now dependent upon others not legally bound for his support; that the mother of said son died at Johnson Co., Ill. On the 28th day of November, A.D. 1851. That he has not heretofore received nor applied for pension. Declarant hereby appoints, with full power of substitution and revocation, John. S. Crum, of Vienna, Johnson Co., Ill. true and lawful attorney, to prosecute his claim; that his residence is Johnson Co., Ill. and his Post Office address is Ozark, Johnson Co., Ill.

Also personally appeared William P. Whittenberg, residing in Vienna Ill., and John S. Bridges, residing in Vienna, Ill., persons whom I certify to be respectable and entitled to credit, and who being by me duly sworn, say they were present and saw Hezekiah L. Lambert, the claimant, sign his name, or make his mark, to the foregoing declaration; that they have every reason to believe, from the appearance of said claimant and their acquaintance with him that he is the identical person he represents himself to be; and that they have no interest in the prosecution of this claim.

In the matter of the.application of Hezekiah L. Lambert, Father of Wm Lambert late of Co. F, 31st. Reg Ill. Vols. personally came before me, a Notary Public in and for aforesaid County and State, Josiah Whitnel, New Burnside Ills., and M. M. Sullins, Citizen of the Town of Ozark, County of Johnson, State of Illinois, who are personally known to me to be reputable and entitled to credit, and who, being by me duly sworn, declare as follows:
We have.known the said Hezekiah L. Lambert for 27 years, and we also well knew the said William Lambert in his lifetime, and from common repute in the country, we know that he was killed at Vicksburg, Miss., July 4, 1863. And as neighbors and acquaintances of the family, we.can truthfully say that we have every reason to believe that the said Hezekiah L. Lambert is the father of the said William Lambert. That said William Lambert died unmarried, leaving neither widow nor minor child surviving him.
That said Hezekiah L. Lambert has now no means of support other than his own labor, or the contributions of others not legally bound to support him. That the mother of said soldier has been dead for many years. That we know said statement to be true of our own personal knowledge, derived from personal acquaintance and near residence as neighbors. We further declare that we have no interest in said case, and are not concerned in it's prosecution, and are not related to said applicant.
