Does this person require to be usually of often restrainedby any mechanical appliance, such as a strap, strait-jacket, &c? and if yes, state the character of the applianceused.

8. Has this person ever been an inmate of any hospital or asylum for the insane?If yes, name the said hospital or asylum.

9. What has been the total length of time spent by him (or her) during lifein such asylums?

10. Is the person also epileptic, suicidal or homicidal?
