14 day of June 1920
Henry Vandike who being duly sworn, declares that he is 80 years of age, and a resident of Carrier Mills of County of Saline, State of Illinois; and that he is the identical person who was ENROLLED at Hammondsville, Ohio, under the name of Henry Vandike, on the 25 day of April 1861 as a private in Co. K 3 Ohio Infantry in the service of the United States, in the Civil War and was HONORABLY DISCHARGED at Columbus Ohio, on the 20 day of September 1864 and that he served as Sergeant. That his personal description at enlistment was as follows: Height 5 feet 10 inches; complexion light; color of eyes, gray; color of hair, dark; Occupation was brick molder. That he was born March 11, 1840, at Jefferson Co. Ohio. That the nature of his disability is; he is now practically blind and needs an attendant. That his places of residence since leaving the service have been as follows: Pittsburgh PA, 64 to 66, Fremont Ohio, 66 to 84, Saline and Williamson County, Illinois to Present.
