CERTIFICATE OF DEATH

| The law of 1903 requires that physicians, coroners and midwives shall report deaths to the State Board of Health on the blank forms prescribed by the Board, except in cities to which burial permits are required. In these cities reports shall be made to the proper local official. A fee of 25 cents will be paid for each report made to the State Board of Health. Penalty for not making reports within 30 days, fine of $10 to $100, or imprisonment in jail for 30 days, or both. |
