On this the 26 day of February, A. D. 1891, personally appeared before me Clerk of the Court within and for the county and state aforesaid Theodore L. Reuter, aged forty-six years, a resident of Nashville, Ill. who being duly sworn according to law declares that he is the identical Theodore L. Reuter, who was enrolled on the eighth day of August A. D. 1862 in Company H, 117 Regiment of Illinois Infantry in the War of the Rebellion and served at least ninety days, was honorably discharged on the fifth day of August, 1865. That he has not been in the Military or Naval Service of the United States since August 1865. That he is unable to earn a support by reason of being seriously afflicted with and suffering from Herina on Rupture, disease of bowels, and malarial poisoning.
 
That said disabilities are not due to his vicious habits and are to the best of his knowledge and belief permanent. That he has never applied for a pension. That he makes this declaration for the purpose of being placed on the pension roll of the United States, under the provision of the Act of Congress of June 27th, 1890. He hereby appoints Newton F. Jones of Nashville, Illinois his true and lawful agent to prosecute his claim. the fee to be $10.oo as prescribed by by law. That his Post Office address is Nashville, Illinois.
